Incident Report


The purpose of the incident reporting process is to record incidents which occur as part of participation at the Mission Granite Club and action outcomes if needed. The intention of the incident report is to provide members with a process to address injuries or interpersonal incidents that cannot be resolved.

Please ensure all injuries or accidents have at least one incident report submitted to the league or bonspiel captain.

For interpersonal incidents or conflicts please first consider the following questions:

  • Have you spoken to your league captain or bonspiel captain? 
  • Has every attempt been made to resolve any and all aspects of the incident?

If you do not feel comfortable speaking with a captain or have made attempts to resolve the incident first, please fill out this form and submit the form by either

  1. Handing it in person/emailing it to the night or bonspiel captain
  2. Placing into one of the locked submission boxes at the club

The locked submission boxes will be checked regularly.
